I work in the Geriatrics Division of a large, high quality multispecialty group practice. My practice may be described as ” Extreme Geriatrics”, with focus on the 1-2 % of our patients that are responsible for 20+% of costs. I work with their primary care physicians and a team of professionals to provide in home visits and intensively coordinated care with engagement of patient, family, friends, PCP’s, specialty doctors, social workers, care managers, visiting nurses, physical and occupational therapists, DME specialists, We have been very successful at interrupting cycles of ER hospital rehab home, helping people to stay longer in their homes.
I am interested in how personal health data may be used to better predict and prevent risks of deterioration, how to provide better in person or virtual care for patients who are homebound or nearly so, and how to bring the best technologies to groups of patients who have been traditionally marginalized or ignored .Categories: Individual Members
